Nominations are open to any team or individual currently working for the civil service (nominations can involve partnerships with non civil service organisations).

Each nomination consists of three questions:

• overview of the nomination (200 words max)
• how the nomination has fulfilled the award criteria (400 words max)
• feedback from customers and/or service users (200 words max)

You can self-nominate or nominate a colleague or team.

You can make as many nominations in as many categories as you wish.

Please ensure your entry follows the specific award criteria provided.

Make sure you include what evidence you have to support your application. If you can, provide supporting testimonials.

Please note your entry must be narrative and we do not accept attachments.

Your nomination should focus on the work being carried out in the past year (July 2009 until now). This includes projects that have been started, ongoing or completed within this timeframe.

Each nomination must include the email address of a senior civil servant who is supporting the entry.
